Before getting started, please consider whether or not you can answer “yes” the following question:
- Do you have a referral letter from your primary physician?
To ensure that your first visit with the doctor is spent most effectively, please be sure to fill out our New Patient Information Kit through the Patient Portal. To help us get acquainted with your pain prior to your visit, the kit asks questions such as: How long have you had pain? Were you previously treated by another pain center? Have you visited Center For Pain Management in the past? What type of insurance do you have?
Please either fill the form out directly on the Patient Portal or download, print and bring it with you.
At your initial evaluation
You will be seen by Dr. Samyadev Datta and our medical assistant may also be present. The consultation is likely to last 30-45 minutes. Dr. Datta will conduct a comprehensive review which entails taking a detailed history, medication review, examination and organizing relevant investigations if needed. After a working diagnosis is made, he will formulate a detailed management plan which will be sent to your referring physician and any other specialist involved in your care as a clinical letter.
After an initial evaluation, the doctor will recommend a pain treatment plan
Center For Pain Management uses various approaches for the management of pain including Interventional Pain procedures. This is a group of minimally invasive surgical procedures typically done as an outpatient to alleviate acute, chronic or cancer-related pain conditions. Effective pain management involves honest communication between the doctor, the patient and any family members or caregivers helping the patient. Please remember, each patient is different and what works for one may not work for another.
At Center for Pain Management, we blend advanced medical knowledge with modern treatment methods and interventions to improve your pain. Because chronic pain is so complex, multiple modalities will provide the best results. At Center for Pain Management, we implement the “step process” to goals such as increased comfort, improved physical function, family support and getting back to work.
